NHTD
Nursing home-level care for those with a disability
Nursing Home Transition and Diversion (NHTD) is a Medicaid waiver program suitable for elderly and disabled patients who need to be looked after 24/7. NHTD caregivers receive extra training to qualify them to provide higher levels of care.
This may be for you if you:
- are 18-65 years of age with a physical disability OR age 65+
- are capable of living in the community
- require nursing home-level care
- have a verified physical disability
TBI
Homecare following traumatic brain injury
A similar Medicaid waiver called TBI (Traumatic Brain Injury) offers the same kind of services as NHTD, specifically for individuals requiring nursing home-level care due to a traumatic brain injury.
This may be for you if you:
- are 18-65 years of age
- are capable of living in the community
- require nursing home-level care
- have suffered a traumatic brain injury
Common questions:
Should I get an aide or CDPAP?
It’s up to you! Some patients like having someone they know look after them, and others prefer a professional whom they don’t know personally. There are also some differences in what services each kind of caregiver can perform. An HHA can help with only basic medical needs — including medication reminders, blood pressure monitoring, and bed transfers — while a CDPAP caregiver can also perform more hands-on medical services, like operating medical equipment, dressing wounds, and administering medication.
Both are excellent options.
What happens if I am unhappy with my aide?
Every patient is assigned a dedicated coordinator who oversees and closely monitors their care. If you are unsatisfied with your care for any reason, your coordinator will step in and solve the problem.

What are all my homecare options again?
There are 2 Medicaid programs that provide homecare services in the state of New York:
● CDPAP (Consumer Directed Personal Assistance Program). You choose your caregiver.
● LHSCA (Licensed Home Care Service Agencies). An agency provides you with an aide.
In addition, there are 2 Medicaid waiver programs that offer more hours and more intensive care:
● NHTD (Nursing Home Transition and Diversion). This is for patients aged 18-65 with a physical disability OR age 65+.
● TBI (Traumatic Brain Injury). This is for patients aged 18-65 who have experience brain injury.
